Collecting and using personal information

HR Orthopaedics collects basic information for internal use within the company and the hospital. This includes (but is not limited to) telephone numbers, postal addresses, email addresses, dates of birth. This is entered into a number of different online systems within the hospital including Ultragenda, CRIS Radiology system and Bluespier administrative system. This information may be collected over the telephone when you call the office or via CHG / Orthteam booking lines or in person at the time of outpatient attendance.
We collect your first and last names, date of birth, email address, telephone number when you send us a request regarding our services.  When you use our media services, your information is kept strictly confidential. The security of your personal information is important to us.  We only use your information for the primary purpose of contacting you, creating your online accounts with us, assessing your needs and responding to your patient requests and requirements.  When you enter sensitive information on our forms, this is stored within the hospital administration system and any paper copies are filed appropriately and held within locked office spaces.
In certain situations, HR Orthopaedics may be required to disclose personal data in response to requests by other clinicians within CHG.  Credit card information to make payment for invoices may also be taken over the telephone.
